A proven method to establish a Patient Family Advisory Council (PFAC) or engage patients and families in improvement.  This six month learning collaborative helps you utilize existing resources in your organization and pairs them with tools, resources, and a learning community to integrate the patient voice in your organization.

Gateway Learning Objectives

 

The next Gateways Collaborative Learning Session Dates:

The PFCCpartners Gateways Learning Collaborative has provided an active process for developing authentic and sustainable PFACs in a variety of healthcare organizations including QIN-QIOs, National Beneficiary Family Advisory Council, Outpatient and Community Health Care practices, Ambulatory settings, Academic Hospitals, Critical Access Hospitals, Integrated Health Care Systems.

 

Your team will attend five web-based learning sessions, with action periods in between each session.  During these action periods your team will utilize resources, materials, and the learning community to complete assignments in your own organization.

Gateways to PFACs Planning Tool 

This tool has been developed to provide support to hospitals and healthcare organizations as they prepare to begin the Gateways to Patient Family Advisory Councils Learning Collaborative, presented by PFCCpartners

 

“In 2015 the Sutter Health Sacramento Sierra Region implemented the PFCCpartners Gateways program to bring patients into our Moore Grant clinical initiative work. Bringing the patients onto our team was a cultural transformation for our organization. The patients shared their stories, their experiences and their wisdom. The stories opened our eyes to see what we could not see. The patients helped to direct and focus our work on ICU mobilization to create a patient brochure to enlighten patient and family members to ICU acquired delirium. Our patients also participated in our Hospitalists Retreat.  Patients and physicians partnered to discuss options and suggestions for improving patient experience  scores for physician. This partnership evolved into engaging a patient onto the Hospitalists Steering Committee. This work, which started with the Gateways program, demonstrated that through partnerships we can work better and stronger together.”

Gateways Community of Practice Information Guide 

Gateways Community of Practice is an additional resource for healthcare organizations who have completed the Gateways to Patient Family Advisory Council Learning Collaborative with PFCCpartners. It is a private LinkedIn Group where members will have access to resources & shared learning from PFCCpartners and other organizations to help support the sustainability of patient family engagement efforts. Members are also encouraged to post their own resources, news, comments, and questions relating to the Gateways Collaborative.
