A proven method to establish a Patient Family Advisory Council (PFAC) or engage patients and families in improvement. This six month online learning collaborative helps you utilize existing resources in your organization and pairs them with tools, resources, and a learning community to integrate the patient voice in your organization.

Your team will attend five web-based learning sessions, with action periods in between each session. During these action periods, your team will utilize resources, materials, and the peer learning community to complete assignments in your own organization. The aim of the Gateways Learning Collaborative is to utilize the change package provided to implement a high impact and sustainable patient family advisory councils or programs in your organization.

The PFCCpartners Gateways Learning Collaborative has provided an active process for developing authentic and sustainable PFACs in a variety of healthcare organizations including QIN-QIOs, National Beneficiary Family Advisory Council, Outpatient and Community Health Care practices, Ambulatory settings, Academic Hospitals, Critical Access Hospitals, Integrated Health Care Systems.

Through a partnership with HealthInsight HIIN, PFCCpartners is pleased to offer a Gateways Collaborative to hospitals belonging to the HealthInsight HIIN.  All associated fees are covered by HealthInsight HIIN.

HealthInsight 2018 Gateways Application

 “In 2015 the Sutter Health Sacramento Sierra Region implemented the PFCCpartners Gateways program to bring patients into our Moore Grant clinical initiative work. Bringing the patients onto our team was a cultural transformation for our organization. The patients shared their stories, their experiences and their wisdom. The stories opened our eyes to see what we could not see. The patients helped to direct and focus our work on ICU mobilization to create a patient brochure to enlighten patient and family members to ICU acquired delirium. Our patients also participated in our Hospitalists Retreat.  Patients and physicians partnered to discuss options and suggestions for improving patient experience scores for physician. This partnership evolved into engaging a patient onto the Hospitalists Steering Committee. This work, which started with the Gateways program, demonstrated that through partnerships we can work better and stronger together.”
